About The Position

Reporting to the Director of Care (DOC) or Director of Health and Wellness (DHW), the Registered Practical Nurse (RPN) is responsible for providing planned, patient‑oriented, goal‑directed nursing care in accordance with provincial standards of care. Shifts Available: Part-Time / Casual / Temporary Part-Time: 30 hours biweekly, Days — 6:15 a.m. to 2:15 p.m. 30 hours biweekly, Evenings — 2:15 p.m. to 10:15 p.m. Temporary Part-Time (0.2 FTE): 1 day per week — 15 hours biweekly

Requirements

  • Current registration as a Registered Practical Nurse in the applicable province/state.
  • Current Basic Cardiac Life Support and First Aid certification required.
  • Proficiency with Microsoft Office applications.
  • Current (within 6 months) Vulnerable Position/Sector Screening (VPS).

Nice To Haves

  • Experience working in a seniors’ environment and with unregulated care providers preferred.
  • Education and experience in gerontology, rehabilitation, community/complex/long‑term care an asset.
  • Strong written and oral communication, critical thinking, care coordination, time management, and customer‑focused skills.
  • Demonstrated sound clinical knowledge.

Responsibilities

  • Work as a member of the interprofessional team as described by Extendicare’s Professional Practice Model, applying a person‑centered approach to care.
  • Complete and ensure resident clinical care assessments, care/service plans, and documentation are accurate, up to date, and communicated as required.
  • Implement and evaluate the effectiveness of care/service plans and make changes as needed.
  • Assist with scheduling resident care conferences and participate as applicable.
  • Work closely with DOC/DHW to facilitate resident move‑ins, move‑outs, and transfers.
  • Provide direct resident care and medication/treatment administration, documenting care in compliance with regulatory requirements and Extendicare Resident Care standards, policies, and procedures.
  • Process orders, safely store drugs and narcotics, and administer medications in accordance with regulatory requirements, scope of practice, and Extendicare policies.
  • Facilitate and provide resident/family education.
  • Plan, coordinate, manage, and evaluate residents’ care plans in collaboration with residents/substitute decision‑makers, physicians, nursing and interdisciplinary teams, and contracted service providers.
  • Coordinate nursing team resident care assignments to ensure efficient and timely service delivery.
  • Respond to and manage significant changes in resident condition and medical emergencies; provide emergency first aid as required.
  • Teach, delegate to, and direct unregulated care providers.
  • Respond to resident/family concerns and ensure appropriate follow‑up, communicating concerns to DOC/DHW/Executive Director and other departments as needed.
  • Assist the DOC/DHW with coordination, implementation, and monitoring of Infection Prevention and Control, Health and Safety, and Clinical Programs, as well as Quality/Safety Plans for the Nursing Department.
  • Uphold and promote the organization’s values and philosophy regarding ethics, morality, and integrity as outlined in Extendicare’s Code of Conduct.
  • Complete all other duties as assigned.
