Position Summary: Provide hemodialysis treatments for chronic kidney disease patients in an outpatient clinic setting Collaborate with the multi-disciplinary team to develop, implement, and monitor effectiveness of individualized plans of care Responsible and accountable for decisions and actions in the provision and management of Nursing care for patients in a culture of “care partnership model” Responsible for the provision of nursing care, teaching and coaching to clients and their families Responsible and accountable for the provision of nursing care in accordance with the standards of nursing practice and THP Best Practice Protocols This position will transfer to the Speakman and Watline satellite sites within 6-12 months of hire

Requirements

  • Candidate will be a diploma graduate of a recognized college or school of nursing or equivalent (i.e., certificate RPN with added courses in medication administration, phlebotomy, and health assessment)
  • Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required
  • Current BCLS
  • Experience in hemodialysis required
  • Minimum of two years recent medical/surgical experience in an acute care setting required
  • Demonstrates knowledge of care for the patient with end stage renal disease
  • Demonstrates knowledge of and experience with the following: wound, skin and foot care; the diabetic patient; venipuncture; routine practices of infection control; normal and abnormal blood values
  • Demonstrates technical ability
  • Previous experience in patient education programs
  • Demonstrates excellence in communication skills and teamwork
  • Demonstrates strong attention to patient and family-centred care and establishing therapeutic relationships
  • A positive work and attendance record is essential

Nice To Haves

  • Completion of Nephrology Nursing Certification courses is preferred.
  • Evidence of ongoing professional development preferred
