About The Position

The Registered Nurse (RN) is responsible for supervising direct care staff, performing direct care according to current standards of practice, state and federal regulations, and facility policy, while overseeing the health and wellness of assigned residents. The role involves communicating professionally and effectively with all members of the care team, including residents and their families.

Requirements

  • RN degree from an accredited nursing program, college, or university.
  • Licensed RN with active license to practice in the state of Texas.
  • Minimum of one year of nursing experience in long-term or other healthcare setting.
  • Assessment and clinical skills (med surg level).
  • Supervisory and communication skills.
  • Excellent customer service skills.
  • Basic computer skills.
  • Current BLS certification.
  • Ability to ensure timely arrival and stable transportation.
  • Ability to work in the environment conducive to caring for residents, with or without reasonable accommodation and without posing a direct threat to self or others.

Nice To Haves

  • 2+ years of experience preferred.
  • Certified Dementia Practitioner Certification preferred.

Responsibilities

  • Supervising direct care staff.
  • Performing direct care according to current standards of practice, state and federal regulations, and facility policy.
  • Overseeing the health and wellness of assigned residents.
  • Communicating professionally and effectively with all members of the care team, including residents and families.
  • Performing admission assessments and interim assessments for residents.
  • Assessing, intervening, and reporting changes in resident's physical/emotional condition, reactions to medications and treatments.
  • Attending and participating in care conferences and reporting to the responsible party or physician as needed.
  • Assessing and monitoring assigned residents' progress and giving feedback on individual interventions with the care team.
  • Performing direct resident care requiring nursing skills, including but not limited to IV, Wound Vacs, Injections, Gastrostomy Feedings, Ostomy care, Tracheostomy Care, and surgical drains.
  • Carrying out and coordinating physician orders in a timely manner.
  • Participating in and completing discharge assessments and required patient/family teaching.
  • Directing and assigning work for direct caregivers (CNA, CMA) to ensure quality care for residents.
  • Communicating clearly and professionally with family, residents, professional providers, and teammates.
  • Requisitioning approved materials, supplies, and equipment to carry out planned/ordered care, which may include medication refills being maintained in a timely manner, checking orders for errors and accuracy.
  • Maintaining a safe environment for residents, including adherence to infection control policies.
  • Completing and maintaining compliance with Relias Education Courses.
  • Performing other assignments and duties as requested.
