About The Position

The Registered Nurse (RN) Charge Nurse is responsible for supervising direct care staff, performing direct care according to current standards of practice, state and federal regulations, and facility policy, while overseeing the health and wellness of assigned residents. This role requires professional and effective communication with all members of the care team, including residents and their families. The RN performs admission and interim assessments, intervenes and reports changes in resident's physical/emotional condition, and participates in care conferences. They also perform direct resident care, coordinate physician orders, and complete discharge assessments. Additionally, the RN directs and assigns work for direct caregivers (CNA, CMA) to ensure quality care, requisitions supplies, maintains a safe environment, and completes required education courses. The position aims to establish and encourage an atmosphere of optimism, warmth, and support for person-centered care.

Requirements

  • RN degree from an accredited nursing program, college, or university
  • Licensed R.N. with active license to practice in state of Texas
  • Minimum of one year in nursing experience in long term or other health care setting
  • Assessment and clinical skills (med surg level)
  • Supervisory and communication skills
  • Excellent customer service skills
  • Basic computer skills
  • Current BLS certification
  • Ability to ensure timely arrival and stable transportation

Nice To Haves

  • 2+ years of nursing experience in long term or other health care setting
  • Certified Dementia Practitioner Certification preferred - course is offered and required once hired

Responsibilities

  • Supervising direct care staff
  • Performing direct care according to current standards of practice, state and federal regulations and facility policy
  • Overseeing the health and wellness of assigned residents
  • Communicating professionally and effectively with all members of the care team, including resident and families
  • Performing admission assessments, interim assessments for residents
  • Assessing/Intervening/Reporting changes in resident's physical/emotional condition, reactions to medications and treatments
  • Attending and participating in care conferences and reporting to responsible party or physician as needed
  • Assessing/monitoring assigned residents progress and giving feedback on individual interventions with care team
  • Performing direct resident care requiring nursing skills; including, but not limited to IV, Wound Vacs, Injections, Gastrostomy Feedings, Ostomy care, Tracheostomy Care, surgical drains
  • Carrying out/coordinating physician orders in a timely manner
  • Participating and completing discharge assessments and required patient/family teaching
  • Directing/Assigning work for direct caregivers (CNA, CMA) to ensure quality care for residents
  • Communicating clearly and professionally with Family, Resident, Professional Providers and Teammates
  • Requisitioning approved materials, supplies, and equipment to carry out planned/ordered care; which may include medication refills being maintain in a timely manner, checking orders for errors and accuracy
  • Maintaining a safe environment for residents, including adherence to infection control policies
  • Completing and maintaining compliance with Relias Education Courses
  • Performing other assignments and duties as requested
