About The Position
Concordia Visiting Nurses is seeking a Registered Nurse to provide personalized, one-on-one home health care in the Ambridge area. The nurse conducts patient assessments and develops individualized care plans in collaboration with physicians, patients, and families. The role includes treatments, IV starts, chest tube management, and wound care, as well as educating patients and families to help them manage health conditions and build confidence in self-care. Nurses monitor patient conditions and identify changes using clinical assessment skills. The position offers autonomy to manage schedules and make independent patient-care decisions, along with support from a collaborative team and opportunities for professional growth.
Requirements
- A minimum of one year of recent clinical experience as a Registered Nurse is recommended.
- Adaptability, compassion, and commitment to patient-centered care.
- Ability to work independently and manage time effectively.

- Home care experience is recommended.
