About The Position

Concordia Visiting Nurses is seeking a Registered Nurse to provide home health care in Ambridge. The role focuses on personalized, one-on-one patient care, building relationships with patients and families, and supporting patients’ health and independence. The Home Health RN conducts assessments, develops individualized care plans with physicians, patients, and families, provides clinical treatments, monitors patient conditions, and educates patients and families about managing health conditions and self-care. The position offers autonomy in managing schedules and patient-care decisions, along with a collaborative team environment and opportunities for training and professional development.

Requirements

• Registered Nurse (RN).
• A minimum of one year of recent clinical experience as a Registered Nurse is recommended.
• Home care experience is recommended.
• Adaptability, compassion, and commitment to patient-centered care.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct thorough patient assessments and develop individualized care plans in collaboration with physicians, patients, and families.
• Provide treatments, including IV starts, chest tube management, and wound care.
• Educate and empower patients and families to manage health conditions and build confidence in self-care.
• Use clinical assessment skills to monitor patient conditions and identify changes.
• Manage a patient schedule and make independent decisions regarding patient care.
