About The Position

Mayo Clinic is seeking a Registered Nurse for Charter House Short-Term Rehabilitation. Charter House is Mayo Clinic’s retirement community located in Rochester, Minnesota, offering Assisted Living and Short-Term Rehabilitation Centers. The Short-Term Rehabilitation Care Team plans personalized care, monitors progress, and adjusts plans to help residents recuperate. This role involves delivering person-centered nursing care, monitoring and managing residents’ health needs, collaborating with interdisciplinary teams, and accurately documenting skilled interventions.

Requirements

Graduate of an Accreditation Commission for Education in Nursing (ACEN) accredited associate's degree or diploma program.
If graduated from a nursing program that was not accredited by ACEN, at least one year of RN experience (within the last five years) in an applicable care setting is required.
If graduation did not occur within the last two years, one year of RN experience in an applicable care setting or one year of current LPN experience at Mayo Clinic is required.
Active Minnesota Registered Nurse License
Basic Cardiac Life Support Certification for Health Care Providers (certified by American Heart Association, Red Cross, or Military Training Network).

Nice To Haves

One year of work experience in Long Term Care or Home Health or two years of acute care nursing is preferred.
Ability to adapt to a dynamic environment and prioritize resident needs.
Ability to work independently.
Strong commitment to teamwork, communication, and resident-centered care.
Excellent communication and human relation skills to interact with Health Service and Charter House staff.
Strong problem-solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process for groups of residents and employees.
Ability to adapt to unpredictable situations and stress.

Responsibilities

Deliver person-centered nursing care in alignment with Mayo Clinic standards.
Assess, diagnose, identify outcomes, plan, implement, and evaluate care.
Monitor and manage residents’ health needs across multiple care settings.
Deliver direct resident care and supervise nursing care provided by the professional and ancillary nursing team across care settings including Individualized Health Services, Catered Living, and Enhanced Living.
Collaborate with interdisciplinary teams to ensure resident safety and well-being.
Assess and plan in collaboration with the resident, family, and the interdisciplinary health care team, to meet the care, educational, and discharge needs.
Accurately document skilled interventions to reflect clinical judgement and resident needs.
