About The Position

The Registered Nurse Case Manager manages patients’ care plans from admission through discharge and ensures the delivery of quality patient care. The role combines direct and indirect patient care, directs staff nurses, LPNs, and CHHAs in delivering each patient’s plan of care, identifies interdisciplinary needs, and coordinates allied health clinicians. The position covers Shelby, Tipton, and Fayette Counties, Tennessee, and is paid on a per-point basis; the listed annual compensation is an estimate.

Requirements

• Graduate from an approved school of professional nursing.
• Hold a current license to practice as a registered nurse in the state where the agency operates.
• Have at least one year of experience as an RN.
• Hold current CPR certification.

Responsibilities

• Assess the nursing needs of assigned patients and manage their overall care plans.
• Complete initial patient assessments upon admission, including obtaining an appropriate patient history.
• Select appropriate nursing diagnoses.
• Assess medicines for potentially ineffective therapy, adverse reactions, significant side effects, drug allergies, and contraindications.
• Establish realistic, measurable, observable goals consistent with each patient’s diagnosis.
• Inform the physician of assessment results and recommended clinical interventions.
• Implement appropriate nursing interventions consistent with the patient’s diagnosis, established goals, and the scope of the Nurse Practice Act.
• Re-evaluate patients’ nursing and other clinical needs as required.
• Document and submit assessment, progress, and discharge notes and other required paperwork in a timely manner.
• Inform the physician and other personnel of changes in the patient’s condition and needs.
• Direct staff nurses, LPNs, and CHHAs in delivering the individual patient’s plan of care.
• Identify interdisciplinary needs and coordinate allied health clinicians.
