About The Position

FlexStaff’s client is seeking a dedicated Registered Nurse to serve as an Assisted Living Facility Nursing Case Manager in Nassau County, New York. The role offers a slower-paced, more personal nursing environment focused on proactive health management and meaningful, long-term relationships with residents. Residents are generally stable and require skilled nursing care, but rarely need critical or emergency interventions.

The nurse provides holistic, person-centered care through comprehensive assessments, individualized care planning, medication and treatment administration, resident and family education, and coordination with families, physicians, and an interdisciplinary team. The position follows a four-day workweek, with consistent daytime hours from 7:00 a.m. to 5:30 p.m. and a flexible schedule that includes weekend rotation.

Requirements

• Hold an active Registered Nurse (RN) license in New York.
• Must have a BSN.
• Demonstrate compassion, empathy, and a patient-centered approach.
• Communicate effectively and build rapport with residents, families, and staff.
• Proactively identify resident needs and develop solutions.
• Have skills in clinical assessment and care plan development.

Responsibilities

• Perform thorough assessments to understand each resident’s unique needs.
• Develop, implement, and evaluate individualized care plans.
• Administer medications and treatments.
• Educate residents and their families on health management and wellness.
• Collaborate with the interdisciplinary team to support optimal resident outcomes.
• Advocate for residents’ health and comfort.
• Coordinate with families and physicians to support seamless, person-centered care.
