About The Position

Requirements

• Nursing Diploma required, ASN preferred.
• License and current registration to practice as Registered Professional Nurse in New York State
• Current NYS Driver’s license
• BLS - Basic Life Support - American Heart Association (AHA)
• RN - Registered Nurse - New York State Education Department (NYSED)
• M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

Nice To Haves

• Bachelors degree of Science in Nursing preferred.
• 1 year of experience working with a frail or elderly population preferred.
• Availability of Motor Vehicle
• 6 months- 1 year experience conducting UAS-NY Community Assessment preferred.

Responsibilities

• Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care
• Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
• Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff
