About The Position

DaVita is seeking an organized, self-directed registered nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in-clinic so they can perform dialysis independently at home, and supports patients through assessments, care planning, treatment monitoring, home visits, and on-call availability. The schedule is Monday through Friday in 8-hour shifts, with weekends and holidays off. Flexible scheduling is available to manage the caseload.

Requirements

• RN license in the state of practice.
• Valid driver’s license, insurance, and reliable transportation.
• Associate degree in nursing (ADN) required.
• Basic computer skills, including MS Word and Outlook.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• Dialysis experience, including PD, HHD, chronic, or acute dialysis.
• Patient education or teaching background.
• Case management or telehealth experience.
• CNN or CDN certification.

Responsibilities

• Train patients and caregivers on peritoneal dialysis procedures and self-care.
• Manage a caseload of PD patients, including assessments and care planning.
• Monitor treatment progress.
• Conduct home visits.
• Provide on-call support.
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
