About The Position
DaVita is seeking an organized, self-directed Registered Nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, manages a PD patient caseload, monitors treatment progress, conducts home visits, and provides on-call support. The position is based at 705 28th St, Grand Rapids, Michigan.
Requirements
- Hold an RN license in the state of practice.
- Have a valid driver’s license, insurance, and reliable transportation.
- Have an ADN.
- Have basic computer skills, including MS Word and Outlook.
Nice To Haves
- A BSN.
- Dialysis experience, including PD, HHD, chronic, or acute dialysis.
- A background in patient education or teaching.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on peritoneal dialysis procedures and self-care.
- Manage a caseload of PD patients, including conducting assessments and developing care plans.
- Monitor treatment progress, conduct home visits, and provide on-call support.
- Collaborate with interdisciplinary teams to support safe, high-quality outcomes.
