About The Position

DaVita Kidney Care is expanding its services by bringing industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services (DSD). This initiative aims to reduce the burden on SNF patients who are transported for dialysis and lower healthcare costs. DSD Services teammates will leverage DaVita's resources to develop infrastructure and grow this new service line. The ideal candidate will have experience in dialysis, thrive in a start-up environment, be passionate about improving patient lives, and possess a proven ability to Get Stuff Done (GSD). Candidates must be comfortable working autonomously and collaborating with cross-functional teams. In this role, the Registered Nurse (RN) is responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. DSD RNs will troubleshoot and make sound decisions regarding patient dialysis treatments, occasionally partnering with SNF staff for care coordination. The primary work schedule is 3-4 days a week, with some travel to other SNF clinics in the region as needed. The impact on patients and their families will be significant.

Requirements

  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 12 months of RN experience
  • Associates Degree in Nursing (A.D.N) from accredited school of nursing required
  • Basic computer skills and proficiency in MS Word and Outlook

Nice To Haves

  • Previous Chronic Hemodialysis, or Acute Dialysis experience is preferred but not required.
  • Training/Teaching/Healthcare Education background
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN)

Responsibilities

  • Take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track.
  • Troubleshoot and make sound decisions for patients related to their dialysis treatment.
  • Occasionally partner alongside the SNF staff to coordinate care.
  • Conduct impactful and meaningful education to patients and their families in the SNF setting.
  • Independently demonstrate best practice techniques and deliver optimal dialysis care.
  • Supervise Dialysis Technicians who are tasked with delivering tandem care to meet program capacity and efficiency goals.
  • Provide comprehensive patient care in an independent setting.
  • Assess, diagnose, and implement complete care plans, looking at the whole patient.
  • Manage and track patient care needs effectively.
