About The Position
DaVita is seeking an organized, self-directed Registered Nurse to manage Peritoneal Dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in-clinic so they can perform dialysis independently at home, manages a PD patient caseload, monitors treatment progress, conducts home visits, provides on-call support, and collaborates with interdisciplinary teams to support safe, high-quality outcomes. The schedule is Monday through Friday, with eight-hour shifts and weekends and holidays off. Scheduling is flexible to support management of the nurse’s own caseload. The position is based at 1155 W Linda Ave Ste A, Hermiston, Oregon.
Requirements
- RN license in the state of practice.
- Valid driver’s license, insurance, and reliable transportation.
- Associate Degree in Nursing (ADN) required.
- Basic computer skills, including MS Word and Outlook.
Nice To Haves
- Bachelor of Science in Nursing (BSN) preferred.
- Experience in dialysis, including PD, HHD, chronic, or acute dialysis.
- Patient education or teaching background.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on peritoneal dialysis procedures and self-care.
- Manage a caseload of peritoneal dialysis patients, including assessments and care planning.
- Monitor treatment progress.
- Conduct home visits.
- Provide on-call support.
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
