About The Position

Elara Caring is seeking a compassionate Registered Nurse Home Health to provide home-based nursing care and support its mission of delivering the right care at the right time in the right place. The nurse assesses patients' care needs, completes initial and ongoing assessments, provides care according to plans of care and physician orders, monitors patients and reports changes, and educates patients and families. The role also includes protecting privacy under HIPAA and helping ensure care is safe, high quality, and continuous.

Requirements

• Hold a current state license as a Registered Nurse.
• Graduate from an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Have at least 1 year of experience as a nurse in a clinical care setting.
• Be able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Have a valid driver's license and maintain an acceptable driving record, or have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

• Home Health experience is preferred but not required.

Responsibilities

• Assess patients to determine their care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on their care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
