About The Position

The Registered Nurse in Case Management coordinates the interdisciplinary plan of care from admission through discharge. The role develops care plans across the continuum while incorporating patient and family preferences and values. It monitors medical necessity and level of care through assessments, ongoing evaluations, and patient records; advocates for resources and removal of barriers; and maintains communication with the supervisor and care transition team to support implementation and reevaluation of the health plan. The nurse also provides guidance on medical record documentation, level-of-care recommendations, and services related to diagnoses, treatment options, and post-discharge care.

Requirements

Currently licensed or eligible to be licensed as a Registered Nurse by the Alabama Board of Nursing.
Diploma from an accredited school or college of nursing and professional licensure at the time of hire.
One year of RN experience.
BLS Provider certification obtained within 30 days of hire or job transfer; American Heart Association or American Red Cross certification accepted.
