This role assists the primary RN with admissions, discharges, and transfers as needed. The ADT RN for admissions will complete the admission assessment and skin check for all new admits. For discharges, the ADT RN will check in and out with the primary RN, review medication reconciliation, and conduct discharge education with the patient and family. The role also involves completing the discharge note, removing the IV (if needed), and entering transport requests into Tele Tracking.

Responsibilities
