About The Position

Guardian Angel Senior Services is seeking a compassionate, organized Registered Nurse (RN) to join our home care team. This role focuses on helping seniors remain safe at home by supporting the caregivers who make it possible. The RN will utilize their nursing knowledge, communication skills, and commitment to quality care for client assessments, caregiver education, and care coordination.

Requirements

• Current Massachusetts Registered Nurse license.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and collaborate with a team.
• A patient, practical approach to educating caregivers and families.
• Reliable transportation for visits in Billerica and surrounding towns.

Nice To Haves

• Home care, visiting nurse, geriatric, or long-term care experience preferred.

Responsibilities

• Conduct client assessments and identify care needs.
• Develop, review, and update individualized care plans.
• Monitor changes in client condition and communicate concerns promptly.
• Provide guidance and support to caregivers, clients, and families.
• Ensure caregivers follow care plans and agency requirements; provide re-education as needed.
• Assist with caregiver training, competency reviews, and ongoing education.
• Maintain accurate, timely nursing documentation.
• Collaborate with the office team to support safe, consistent care.
