Registered Nurse - Home Health
About The Position
BAYADA at BAYhealth is seeking a Registered Nurse for a full-time or part-time opportunity performing home health visits with adult patients throughout Smyrna, Clayton, and Dover, Delaware. As a member of a multidisciplinary health care team, the nurse provides rehabilitative care and skilled nursing to help clients receive the medical care they need while remaining at home. The role includes visiting clients in assigned geographic territories, carrying out care plans, providing nursing treatments, monitoring client conditions, coordinating care, and documenting services using a touch pad tablet. Base pay is $62–$67 per point, with stated annual earning potential of $96,720–$104,520, depending on qualifications and status and subject to negotiation.
Requirements
- Current Registered Nurse license in Delaware.
- At least one year of recent, verifiable clinical (medical/surgical) experience.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills.
- Ability to travel to assigned cases.
Nice To Haves
- Prior home care experience is strongly preferred but not required.
- Prior experience with electronic medical records (EMR) is preferred.
Responsibilities
- Follow a designated care plan in accordance with each patient's needs.
- Make home visits to clients in designated geographic territories.
- Perform assigned duties, including medication administration, wound care, treatments, and procedures.
- Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
- Follow up on, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
- Provide case management and coordinate care.
- Accurately document observations, interventions, and evaluations related to client care management and services using a touch pad tablet.
