About The Position

Hosparus Health is seeking compassionate Registered Nurses (RNs) to join their team. The RN will be responsible for assessing patient and family needs, planning, implementing, evaluating, and documenting individualized hospice nursing care. This role involves care coordination and collaboration with the Interdisciplinary Group and other medical providers, as well as supervising Licensed Practical Nurses and Certified Nursing Assistants. Patients may be located in home, skilled nursing facility, assisted living facility, or acute care settings. The RN will create moments that matter most for patients facing advanced illness or end-of-life care.

Requirements

• Registered Nurse (RN) license
• Ability to assess and identify patient/family needs.
• Ability to plan, implement, evaluate, and document individualized hospice nursing care.
• Ability to coordinate care and collaborate among the Interdisciplinary Group and other medical providers.
• Ability to supervise Licensed Practical Nurses and Certified Nursing Assistants.
• Willingness to provide care in home, skilled nursing facility, assisted living facility, or acute care settings.

Responsibilities

• Assess and identify patient/family needs.
• Plan, implement, evaluate, and document individualized hospice nursing care.
• Provide care in accordance with the interdisciplinary plan of care and policies of the Clinical Services Department.
• Coordinate care and collaborate among the Interdisciplinary Group and other medical providers.
• Supervise the hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants.
• Provide care to patients in a home, skilled nursing facility, assisted living facility, or acute care setting.
