About The Position
DaVita is seeking an organized, self-directed Registered Nurse to manage Home Hemodialysis (HHD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, manages a caseload of HHD patients, monitors treatment progress, conducts home visits, and provides on-call support. The position works with interdisciplinary teams to support safe, high-quality patient outcomes. The schedule is Monday through Friday in 8-hour shifts, with weekends and holidays off. Scheduling is flexible to allow the nurse to manage their own caseload. The role is based at 390 S Fair Oaks Ave, Suite 120, Pasadena, California.
Requirements
- RN license in the state of practice.
- Valid driver’s license, insurance, and reliable transportation.
- Associate Degree in Nursing (ADN) required.
- Basic computer skills, including MS Word and Outlook.
Nice To Haves
- Bachelor of Science in Nursing (BSN).
- Experience with dialysis, including HHD, peritoneal dialysis (PD), chronic, or acute dialysis.
- Patient education or teaching experience.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on HHD procedures and self-care.
- Manage a caseload of HHD patients, including conducting assessments and developing care plans.
- Monitor treatment progress, conduct home visits, and provide on-call support.
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
