About The Position
At our Skilled Nursing Facility, RNs play a vital role in helping older adults recover, maintain dignity, and live safely - whether they’re here for short-term rehab or long-term memory care. You’ll be part of a supportive, multidisciplinary team that values compassion, teamwork, and consistent quality care. This role involves delivering compassionate care in a skilled nursing facility, focusing on older adults' recovery, dignity, and safety. You will be part of a supportive, multidisciplinary team.
Requirements
- Registered Nurse (RN) in IL
- Electronic medical records experience is necessary (Point Click Care or PCC is ideal)
Nice To Haves
- Prior floor nurse experience within a skilled nursing facility is helpful
- Experience with the behavioral health population is helpful, but not required
Responsibilities
- Provide safe, person-centered nursing care tailored to older adults’ needs
- Administer medications and treatments following individualized care plans
- Monitor resident progress and promptly address changes in condition
- Collaborate closely with CNAs, therapists, and physicians as part of the care team
- Help maintain a calm, respectful, and safe environment for all residents
- Document care accurately using electronic health records (PCC)
