About The Position

Southwoods Health is hiring a full-time Transitional Care Management RN for its clinic at the Main Campus in Boardman, Ohio. The RN assists patients recently discharged from acute care by gathering clinical data and helping prevent hospital readmissions. The schedule is Monday through Friday on the day shift.

Requirements

• Active Ohio RN license.
• Strong critical-thinking skills.
• Strong time-management skills.
• Clear, professional, and empathetic phone skills.

Nice To Haves

• BSN degree preferred.

Responsibilities

• Conduct post-acute phone assessments.
• Identify barriers to patient recovery.
• Review discharge instructions, medications, and referrals.
• Facilitate and schedule face-to-face follow-up visits.
• Accurately document all patient calls in the EMR system.
• Maintain patient privacy and follow regulatory standards, including OSHA and ODH requirements.
