This is a part-time, fully remote at-home position requiring 20 hours per week, primarily on Saturdays and Sundays, with the potential for holiday work. The role involves conducting comprehensive clinical assessments of high-risk patients post-discharge, developing and updating individualized care plans, and coordinating care among various healthcare providers and community resources. The Registered Nurse will serve as the main point of contact for patients and families, providing education and managing a caseload to ensure adherence to treatment plans and follow-up appointments. Collaboration with interdisciplinary teams, monitoring patient progress via telehealth and phone, and ensuring compliance with regulatory guidelines are key responsibilities. The position also involves participation in quality improvement initiatives to enhance transitional care workflows and patient outcomes.
