About The Position

iCare Health Network is seeking a motivated and experienced Registered Nurse (RN) to join our team as a Transitional Care Nurse. This is a unique opportunity for an RN who enjoys combining clinical nursing, patient advocacy, care coordination, and relationship building. The Transitional Care Nurse will work across three iCare locations, supporting patients as they transition between hospitals, skilled nursing, dialysis, specialty care, and home. In this role, you will play an important part in helping patients receive the right care at the right time while working closely with facility teams, physicians, hospitals, patients, and families. This is a travel position between three iCare locations: Westside Care Center – Manchester, CT, Parkville Care Center – Hartford, CT, and Silver Springs Care Center – Meriden, CT. The Transitional Care Nurse will support the dialysis patient population at Westside and Parkville, as well as the VA patient population at Silver Springs. This position provides an opportunity to work with diverse patient populations while collaborating with multiple clinical teams and healthcare partners throughout the community.

Requirements

Current RN license (state-specific, active, and in good standing).
Familiarity with electronic health record systems (e.g., PCC).
Exceptional interpersonal, organizational, and clinical teaching skills.
Ability and willingness to travel regularly to required buildings.

Responsibilities
