Requirements

  • Active and unencumbered South Carolina RN License
  • Current CPR Certification
  • Graduate of an accredited School of Nursing
  • Strong clinical, communication, and leadership skills

  • Nursing Degree from an accredited college or university
  • Training in rehabilitative and restorative nursing practices
  • 2 years or more experience, preferably in a long-term care facility
  • Ability to read, write, speak, and understand the English language

  • Ability to read and comprehend policy and procedure manuals
  • Ability to effectively present information and respond to questions from managers and employees
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations

  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public

  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

  • Provide high-quality nursing care to residents.
  • Administer medications and treatments as prescribed.
  • Monitor resident conditions and communicate changes appropriately.
  • Supervise and support CNA and LPN team members.
  • Maintain accurate documentation and resident records.
  • Collaborate with physicians, therapists, and interdisciplinary team members.
  • Build meaningful relationships with residents and families.
  • Supervise the day-to-day nursing activities of the facility during your tour of duty in accordance with current federal, state, and local standards, guidelines, and regulations.
  • Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities.
  • Participate in developing, maintaining, and updating written policies and procedures.
  • Ensure that reference material is current and recommend written material to assist the nursing service department.
  • Ensure that the Nursing Service Procedures Manual is current and that all personnel comply with it.
  • Make written and oral reports/recommendations to the Director concerning the operation of the nursing service department.
  • Periodically review the department’s policies, procedure manuals, job descriptions, etc., and make recommendations for revisions.
  • Assist in developing methods for coordinating nursing services with other resident services.
  • Ensure that all nursing service personnel are in compliance with their respective job descriptions.
  • Participate in the development, maintenance, and implementation of the facility’s quality assurance program.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Periodically review and update the resident’s written discharge plan.
  • Assist the Director in planning the nursing services portion of the resident’s discharge plan.
  • Meet with the nursing staff and support personnel to plan department services, programs, and activities.
  • Admit, transfer, and discharge residents as required.
  • Complete accident/incident reports as necessary.
  • Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies.
  • Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting.
  • Ensure that a sufficient number of licensed practical and/or registered nurses and certified nursing assistants are available for your tour of duty.
  • Develop work assignments and/or assist in completing and performing tasks.
  • Delegate administrative authority, responsibility, and accountability to each Charge Nurse.
  • Ensure that all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program.
  • Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
  • Make daily rounds to ensure personnel are performing work assignments in accordance with acceptable nursing standards and report findings to the Director.
  • Monitor absenteeism to ensure adequate staffing and report problem areas to the Director.
  • Ensure that established policies and procedures, including dress codes, are followed by all departmental personnel.
  • Review complaints and grievances made or filed by department personnel and make appropriate reports.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
  • Meet with your shift on a regularly scheduled basis to identify and correct problem areas.
  • Review and evaluate your department’s workforce and make recommendations to the Director.
  • Participate in employee performance evaluations, determine staffing requirements, and make recommendations concerning dismissals and transfers.
  • Ensure that departmental disciplinary action is administered fairly.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Report known or suspected incidents of fraud.
  • Ensure that departmental computer workstations are properly logged off or password protected.
  • Inform nursing service personnel of new admissions and ensure rooms are ready.
  • Greet newly admitted residents and escort them to their rooms.
  • Make rounds with physicians as necessary and encourage timely recording and signing of progress notes and orders.
  • Inform the Director when physician visits are not made in a timely manner.
  • Consult with the resident, family, and physician in planning resident care.
  • Notify the attending physician and next-of-kin of changes in resident condition.
  • Visit residents daily to observe and evaluate their physical and emotional status.
  • Ensure that direct nursing care is provided by qualified personnel.
  • Review nurses’ notes for informativeness, descriptiveness, and reflection of resident response to care.
  • Provide direct nursing care as necessary.
  • Start IVs, obtain specimens for lab tests, and take vital signs.
  • Authorize the use of restraints when necessary.
  • Ensure RNs and LPNs comply with medication and supply procedures.
  • Monitor medication passes and treatment schedules.
  • Inspect nursing service treatment areas daily for cleanliness and safety.
  • Ensure residents who are unable to call for help are checked frequently.
  • Meet with residents and/or family members as necessary and report problems to the Director.
  • Assist in arranging transportation for discharged residents.
  • Inform family members of the death of a resident and call funeral homes when requested.
  • Participate in developing, planning, conducting, and scheduling in-service training classes.
  • Assist the In-service Director/Educator in developing annual facility in-service training programs.
  • Assist the Director in planning clinical supervision for nurse aide trainees.
  • Attend and participate in continuing education programs.
  • Assist the Safety Officer in developing safety standards for the nursing service department.
  • Identify, evaluate, and classify routine and job-related functions with potential exposure to blood/body fluids.
  • Ensure personnel participate in appropriate in-service training programs prior to tasks involving potential exposure.
  • Ensure an adequate supply of personal protective equipment is available and used correctly.
  • Monitor personnel to ensure they follow established safety regulations.
  • Ensure nursing service work areas are clean and sanitary.
  • Ensure all resident care rooms, treatment areas, etc., are clean, safe, and sanitary.
  • Ensure personnel follow established handwashing and hand hygiene procedures.
  • Ensure personnel participate in fire safety and disaster preparedness drills.
  • Participate in the infection control program.
  • Ensure personnel follow infection control procedures when isolation precautions are necessary.
  • Participate in developing and implementing procedures for reporting hazardous conditions or equipment.
  • Ensure personnel use safety equipment when lifting or moving residents.
  • Report missing/illegible labels and MSDSs.
  • Recommend equipment and supply needs to the Director.
  • Ensure an adequate stock level of medications, medical supplies, and equipment is maintained.
  • Participate in developing a preventive maintenance program for nursing service equipment.
  • Participate in developing and implementing procedures for the safe operation of all nursing service equipment.
  • Ensure only trained and authorized personnel operate nursing service equipment.
  • Ensure personnel operate nursing service equipment safely.
  • Monitor nursing procedures to ensure efficient use of supplies and avoid waste.
  • Ensure appropriate MSDSs are on file for hazardous chemicals.
  • Participate in the development of resident assessments and care plans.
  • Encourage resident participation in care plan development and review.
  • Review resident care plans for appropriate goals, problems, approaches, and revisions.
  • Ensure all personnel involved in resident care are aware of the care plan and refer to it.
  • Review nurses’ notes to determine if the care plan is being followed.
  • Assist the Resident Assessment/Care Plan Coordinator with MDS, RAPs, and Triggers.
  • Review resident’s medical and nursing treatments to ensure they align with the care plan.
  • Keep abreast of economic conditions and recommend adjustments to nursing services.
  • Report suspected or known incidence of fraud relative to billings, cost reports, kickbacks, etc.
  • Assist with the overall supervision and management of the nursing staff.
  • Maintain all required continuing education/licensing.
  • Remain in good standing with the State Board of Nursing at all times.
