About The Position

Visiting Nurse Association of Florida is seeking a Registered Nurse to provide home health services to patients in Leesburg, Florida. The RN plans, coordinates, and assures quality care for patients and their families. The role includes delivering professional nursing services in clients’ homes, managing primary patients, monitoring changing patient and family needs, and coordinating care with physicians, families, colleagues, and community resources. This is a PRN position.

Requirements

Active Florida RN license.
Minimum one year of nursing experience.
Strong assessment and critical-thinking skills.
Ability to work independently.

Nice To Haves

Home health experience preferred.
OASIS experience preferred.

Responsibilities

Develop work methods and procedures that facilitate high-quality care.
Deliver professional nursing services in clients’ homes and provide case management for primary patients.
Monitor changing needs of clients and their families.
Perform initial assessments, oversee progress toward goals, and monitor clinical outcomes.
Maintain ongoing communication with patients, families, physicians, other disciplines, the Clinical Manager, and office staff.
Participate in staff interdisciplinary conferences.
Develop, implement, and continuously monitor plans of care with patients and/or their representatives.
Complete clinical documentation, including OASIS, evaluations, orders, and care coordination documentation.
Work with physicians and discharge planners while promoting health and educating patients and/or caregivers.
Liaise with community health-related resources, including DME, oxygen, and infusion services.
Interact with patients and families, team members, and other health care professionals.
Participate in ongoing education and pursue continuous personal and professional development.
