About The Position
The Registered Nurse Field Nurse in Home Health is responsible for maintaining quality patient care by ensuring that each patient’s care plan is followed. The role combines direct and indirect patient care and provides care to pediatric, adolescent, adult, and geriatric patients. The nurse directs CHHAs and CNAs in patient-care tasks; assesses patients; develops, evaluates, and modifies care plans; and documents care provided. The position is full-time, based in Weatherford, Texas, and covers Weatherford and Grandbury. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with monthly on-call duty. The role is not remote or virtual. Salary is $89,000 to $100,000, based on experience.
Requirements
- Current Registered Nurse licensure to practice nursing in the state.
- At least one year of experience as a Registered Nurse.
- Current CPR certification.
Responsibilities
- Assess the nursing needs of assigned patients, implement individualized care plans, evaluate their effectiveness, and modify them as appropriate.
- Promote and maintain optimal client care by diagnosing and treating human responses to actual or potential health problems.
- Make the initial nursing evaluation visit and reassess clients’ nursing needs as required.
- Initiate plans of care and necessary revisions.
- Provide services requiring specialized nursing skills within the scope of the Nurse Practice Act.
- Assign, supervise, and teach other clinical personnel, including issuing written paraprofessional orders and providing supervision.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Inform physicians and other personnel about changes in clients’ conditions and needs.
- Counsel and instruct clients and families on nursing and related needs.
- Direct CHHAs and CNAs in patient-care tasks and document patient care provided.
