About The Position

DaVita is seeking an organized, self-directed Registered Nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The RN educates patients and caregivers in the clinic so they can perform dialysis independently at home, monitors treatment progress, conducts home visits, and provides on-call support. The position is scheduled Monday through Friday in 8-hour shifts, with weekends and holidays off and flexible scheduling to manage the caseload.

Requirements

• RN license in the state of practice.
• Valid driver’s license, insurance, and reliable transportation.
• Associate Degree in Nursing (ADN) required.
• Basic computer skills, including MS Word and Outlook.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Experience with dialysis, including PD, HHD, chronic, or acute dialysis.
• Background in patient education or teaching.
• Case management or telehealth experience.
• CNN or CDN certification.

Responsibilities

• Train patients and caregivers on peritoneal dialysis procedures and self-care.
• Manage a caseload of peritoneal dialysis patients, including assessments and care planning.
• Monitor treatment progress.
• Conduct home visits.
• Provide on-call support.
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
