About The Position

Nightingale Visiting Nurses is seeking a dedicated Registered Nurse to provide high-quality, one-on-one home health care to patients in Stamford and Norwalk, Connecticut. The role offers autonomy, flexible scheduling, clinical support, and per-visit compensation starting at $55 per visit, with rates based on experience. The nurse helps patients remain safe and independent in their homes by delivering compassionate care and coordinating with an interdisciplinary team.

Requirements

• Current Registered Nurse (RN) license in Connecticut.
• Physical exam within the last 12 months.
• TB test within 12 months and current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to attend a 1–3 day orientation at the Stamford office.

Nice To Haves

• OASIS and WellSky (Kinnser) experience.
• Medical-surgical and wound care experience.

Responsibilities

• Conduct patient admissions and develop individualized Plans of Care.
• Obtain medical histories and perform physical assessments.
• Monitor patient status, vital signs, and clinical progress in the home setting.
• Deliver skilled nursing care in compliance with HIPAA and agency standards.
• Evaluate patient outcomes and adjust care plans as needed.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
• Coordinate services with LPNs, HHAs, and therapy providers.
• Complete accurate documentation and submit clinical notes in a timely manner.
