About The Position
PruittHealth @Home is hiring a Registered Nurse for its Home Health Services Episode Assessment Coordinator role. The position is remote and applicants must live in South Carolina, North Carolina, or Georgia. The role supports home health patients by completing evaluations, assessments, treatments, and documentation in accordance with policy and individualized care plans, while maintaining continuity of care. Applicants must have an active ICD-10 certification and current, active, unrestricted RN licensure in the state of practice.
Requirements
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
- Active ICD-10 certification.
- Must live in South Carolina, North Carolina, or Georgia.
Responsibilities
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
- Complete all documentation and records in a timely and accurate manner.
- Follow established policies and procedures related to the scope of practice.
- Maintain continuity of patient care in a home care setting.
