About The Position

The wound care nurse applies the Nursing Process to the provision of care for patients with acute & chronic wounds, ostomies, as well as patients undergoing hyperbaric oxygen therapy (HBOT) where applicable. The wound care nurse coordinates treatment plans and collaborates with interdisciplinary teams to achieve optimal outcomes within both the inpatient and outpatient setting. Additionally, the wound care nurse maintains comprehensive clinical documentation, adheres to safety protocols, supports and provides patient education and advocacy.

Requirements

  • Applies the Nursing Process to the provision of care for patients with acute & chronic wounds, ostomies, as well as patients undergoing hyperbaric oxygen therapy (HBOT) where applicable.
  • Coordinates treatment plans and collaborates with interdisciplinary teams to achieve optimal outcomes within both the inpatient and outpatient setting.
  • Maintains comprehensive clinical documentation.
  • Adheres to safety protocols.
  • Supports and provides patient education and advocacy.
  • Performs patient assessments, implements care plan, adheres to established standards of care.
  • Assists in the development of wound and ostomy care policies and procedures.
  • Able to assume super-user status when implementing new processes/equipment.
  • Actively interfaces & assists in maintaining a collaborative relationship.
  • Collaborates closely with physicians, APPs, fellow wound care nurses, wound program leadership, and other hospital staff to ensure service excellence.
  • Coordinates with Manager in maintaining a safe environment of care.
  • Adheres to OSHA, CMS, CDC, Joint Commission, NJ Department of Health, and applicable medical center standards/policies.
  • Identifies, coordinates, and meets patient care needs including but not limited to home care, DME orders, communication with all providers involved in the patients care.
  • Serves as a clinical resource to the health care team, patients, and their families.
  • Acts as a mentor/preceptor.
  • Acts as an educator and consultant to staff nurses, physicians, APPs, and support staff for wound, skin and ostomy care concepts, practices, products and devices.
  • Participates in hospital and system-wide research and educational programs and studies.

Responsibilities

  • Collaborates with the Manager in areas of daily operational and fiscal management. Assists in the development of wound and ostomy care policies and procedures. Able to assume super-user status when implementing new processes/equipment.
  • Actively interfaces & assists in maintaining a collaborative relationship. Collaborates closely with physicians, APPs, fellow wound care nurses, wound program leadership, and other hospital staff to ensure service excellence.
