About The Position
Trinity Health IHA Medical Group is seeking a compassionate and patient-focused Registered Nurse (RN) to join their outpatient care team. In this role, the RN will help patients navigate their healthcare journey through clinical triage, care coordination, patient education, and ongoing support that improves outcomes and enhances the patient experience. The RN II performs at an advanced level. This role may also involve conducting nurse visits with patients and/or groups, providing care coordination for patients with complex needs, and/or taking on additional clinical leadership roles. The position also includes precepting and mentoring nursing and clinical support staff.
Requirements
- Graduate of an accredited Registered Nurse program.
- Valid, unrestricted RN license in the State of Michigan.
- Valid CPR certification.
- 2 years of clinical experience in an ambulatory practice as an RN and/or demonstrated competency in IHA’s RN position.
Responsibilities
- Performs high-level nursing care, wellness care, specialty-specific and chronic disease education/management.
- Provides direct care with individual patients and/or populations.
- Provides care coordination for patients with complex needs.
- Assess and triage patient needs by phone and in-office visits.
- Provide clinical guidance, patient education, and care coordination.
- Support patients through transitions of care and post-discharge follow-up.
- Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
- Manage medication refills, test results, referrals, and prior authorizations.
- Identify preventive care needs and support chronic disease management.
- Help patients access community resources and navigate barriers to care.
- Serve as a clinical resource and leader within the care team.
