As a Registered Nurse Behavioral Health, you’ll contribute to our success in the following ways: Assess patients' physical, emotional, social, and environmental needs. Manage a caseload and develop, coordinate, and update individualized care plans. Complete initial assessments and provide ongoing evaluations during patient visits. Monitor patient progress and communicate changes with physicians, supervisors, and the care team. Update care plans based on patients' changing needs. Educate patients, families, and caregivers about the plan of care and behavioral health support. Teach behavioral health care and symptom management to staff in partner care settings. Supervise and support Behavioral Health Aides and LPNs while ensuring quality care. Report safety concerns or other issues to supervisors as needed. Provide compassionate end-of-life support and complete bereavement assessments.

Requirements

  • Current State license as a Registered Nurse.
  • Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
  • 1+ year of experience as a nurse in a clinical care setting.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Nice To Haves

  • Home Health experience is preferred, but not required.
