This Registered Nurse Behavioral Health position serves the Plainfield, Joliet, Naperville, and Bolingbrook areas. It is a Full Time position with Day shifts and No Weekends. The role offers many benefits and requires Behavioral Health Nursing Experience. We are looking for a compassionate and committed individual to join our caring and supportive team and help keep clients in their homes.
Requirements
- Current State unrestricted license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
- 1+ year of experience as a nurse in a clinical care setting.
- Ability to frequently lift, push, pull and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
Nice To Haves
- Home Health experience is preferred, but not required.
Responsibilities
- Assess patients' physical, emotional, social, and environmental needs.
- Manage a caseload and develop, coordinate, and update individualized care plans.
- Complete initial assessments and provide ongoing evaluations during patient visits.
- Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
- Update care plans based on patients' changing needs.
- Educate patients, families, and caregivers about the plan of care and behavioral health support.
- Teach behavioral health care and symptom management to staff in partner care settings.
- Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
- Report safety concerns or other issues to supervisors as needed.
- Provide compassionate end-of-life support and complete bereavement assessments.
