Requirements

  • Graduate of an accredited school of professional nursing.
  • Current license as a Registered Nurse (RN).
  • 2 years of Home Care experience and 1 year of Supervisory experience.
  • Current CPR certification.
  • Access to reliable transportation.
  • Maintains confidentiality in all aspects of the job.
  • Knowledge of safety and infection control practices.

Responsibilities

  • Performing client Admissions in a timely manner in accordance with agency policies.
  • Developing an individualized care plan for each client.
  • Reports client significant changes to the Administrator, authorized client representative and other team members.
  • Maintains client records and documentation.
  • Delegates tasks to unlicensed personnel based upon their scope of practice where allowable.
  • Performs supervisory visits.
  • Participates in the competency skill assessment of unlicensed personnel.
  • Participates as requested in the agency's Quality Assurance Performance Improvement program.
