Requirements

Graduate of an accredited school of professional nursing.
Current license as a Registered Nurse (RN).
2 years of Home Care experience and 1 year of Supervisory experience.
Current CPR certification.
Access to reliable transportation.
Maintains confidentiality in all aspects of the job.
Knowledge of safety and infection control practices.

Responsibilities

Performing client Admissions in a timely manner in accordance with agency policies.
Developing an individualized care plan for each client.
Reports client significant changes to the Administrator, authorized client representative and other team members.
Maintains client records and documentation.
Delegates tasks to unlicensed personnel based upon their scope of practice where allowable.
Performs supervisory visits.
Participates in the competency skill assessment of unlicensed personnel.
Participates as requested in the agency's Quality Assurance Performance Improvement program.
