About The Position

The Nurse Practitioner, when providing palliative care, attends to patients facing chronic, complex, and/or life-threatening conditions across various care settings. Key duties include conducting assessments, educating patients and families on disease and treatment, and managing symptoms to enhance comfort and coping. Collaboration with Primary Care Physicians (PCPs), specialists, and the assigned Compassus Medical Director is essential, ensuring adherence to practice standards and codes. The role demands a flexible schedule to meet agency needs. For hospice care, the Nurse Practitioner acts as an extension of the interdisciplinary team (IDT) and hospice physician, performing routine recertification and emergency assessments, and delivering educational and evaluative services to patients and their families. This involves supporting patients and families in maintaining comfort and coping, with a flexible schedule to accommodate agency visit requirements, potentially including after-hours emergency visits and/or admissions. In palliative care programs, the Nurse Practitioner also handles outpatient responsibilities such as routine and emergency assessments, bedside patient/family evaluations, assisting with diagnosis and management of acute and chronic conditions, ordering and interpreting lab/radiology tests, prescribing medications (including controlled substances as delegated), ordering treatments and durable medical equipment, and performing other therapeutic measures. They consult with palliative care physicians, inform primary physicians, coordinate care for referrals, maintain accurate patient records for medical practice and reimbursement, comply with collaborative agreements, and assist patients and families with goal setting and advanced care planning. Additionally, the role involves training staff and contributing to the development of clinical practice guidelines.

Requirements

  • Master’s degree in Nursing.
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric.
  • Active NPI number.
  • Maintain active DEA certification if state allows prescription of controlled substances by APPs.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percentage.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
  • Strong written and verbal communications.
  • Ability to understand, read, write, and speak English.
  • Articulates and embraces hospice philosophy.
  • Strong organizational and time management skills.
  • Must be able to work a flexible schedule to include nights and weekends.

  • Current collaborative physician agreement in place (All states except Iowa).
  • Minimum of one (1) year nursing practice or equivalent experience (If providing Hospice Care).
  • Minimum of five (5) years nursing experience (If providing Palliative Care).
  • A minimum of 4 days of availability per month is required (If providing Palliative Care).

Nice To Haves

  • Hospice, oncology, or home health and/or long-term care experience preferred (If providing Hospice Care).
  • Clinical nursing care in oncology, hospice, geriatrics, intensive care preferred (If providing Palliative Care).
  • Board Certification in Hospice and Palliative Care is preferred.

  • Completes routine and emergency recertification visits on each patient entering their 3rd benefit period or beyond (including admissions and transfers) (If providing Hospice Care).

  • Communicates with Medical Director, Attending Physician, DCS and IDT as directed regarding findings from the visits and any measures to alleviate symptoms (If providing Hospice Care).
  • Provides physical and emotional care to support the patient and family in period of crisis (If providing Hospice Care).
  • Prevents unwarranted hospitalizations by meeting patient and family needs in the home (If providing Hospice Care).
  • Performs routine and emergency assessments related to outpatient palliative care visits (If providing Palliative Care - Outpatient).
  • Performs bedside palliative care patient/family evaluation in the patient’s place of residence (If providing Palliative Care - Outpatient).
  • Assists with diagnosis, treatment, and management of acute and chronic health conditions (If providing Palliative Care - Outpatient).
  • Orders, performs, and interprets laboratory and radiology tests within scope of professional practice (If providing Palliative Care - Outpatient).
  • Prescribes medications including controlled substances to the extent delegated and licensed (If providing Palliative Care - Outpatient).
  • Orders treatments and durable medical equipment as indicated (If providing Palliative Care - Outpatient).
  • Performs other therapeutic or corrective measures as indicated, including urgent care (If providing Palliative Care - Outpatient).

  • Supervises nurse trainees, including graduate and advance practice nurses in field placements (If providing Palliative Care).
  • Participates in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees (If providing Palliative Care).
