About The Position

Open Sky Community Services is seeking a Registered Nurse for the South County Integrated Team. This role is ideal for a nurse who prefers working outside of a traditional office or hospital setting and wants to build connections with patients. The position offers a unique opportunity to work in a clinically based, supportive, and nurturing role within residential and community settings, focusing on recovery-based care for individuals with mental illness or dual diagnosis. Adult Community Clinical Services (ACCS) is a Department of Mental Health funded service that delivers evidence-based interventions. The individuals served reside in their own homes, apartments, or group living environments, and the services focus on helping them transition to more independent living by building skills and achieving personal goals. The integrated teams are multidisciplinary and supportive, comprising clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff.

Requirements

  • Registered Nurse licensed in Massachusetts, required.
  • Valid driver’s license and acceptable driving history, required.
  • Excellent assessment, documentation, and verbal communication skills, required.

Nice To Haves

  • Experience with adults with mental health and psychiatric disorders, preferred.

Responsibilities

  • Collaborate with the integrated team to complete thorough physical assessments of health needs.
  • Make referrals to appropriate community physicians/health care providers.
  • Monitor the medical/psychiatric status of individuals served by observing and interviewing them, examining documents, and making recommendations.
  • Provide training and consultation to programs regarding medical issues, procedures, and techniques, and health and wellness practices.
  • Assess and provide individualized education related to a person’s health and disease processes; reinforce wellness and preventative health measures.
  • Review clinical records on a regular basis to ensure adequate services and supports are provided, including any applicable medical interventions, tests, or preventative care as necessary.
  • Provide oversight to the Medication Administration Program and ensure compliance with applicable regulations.
