About The Position

We provide opportunities for nurses that are beyond the four walls of an institutional setting, as you work within residential and community settings, focusing on recovery-based care for people diagnosed mental illness or dual diagnosis. Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served.
Requirements
- Registered Nurse licensed in Massachusetts, required.
- Valid driver’s license and acceptable driving history, required.
- Excellent assessment, documentation, and verbal communication skills, required.

Responsibilities
- Collaborates with the integrated team to complete thorough physical assessment of health needs, referrals to appropriate community physicians/health care providers, and monitor medical/psychiatric status of individuals served by observing and interviewing individuals, examining documents, and making recommendations.
- Provide training and consultation to programs regarding medical issues, procedures and techniques, and health and wellness practices.
- Assess and provide individualized education related to person’s health and disease processes; reinforce wellness and preventative health measures.
- Review clinical records on a regular basis to ensure adequate services and supports are provided, including any applicable medical interventions, tests, or preventative care as necessary.
- Provide oversight to the Medication Administration Program and ensure compliance with applicable regulations.
