About The Position
Home Health Registered Nurse RN Position: Are you a Registered Nurse (RN) with a passion for personalized, one-on-one patient care? Do you thrive on building meaningful relationships and empowering patients? As a Home Health RN, you'll be at the heart of this mission. Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families. Treat patients, perform IV starts, manage Chest Tubes, and provide Wound care. Educate and empower patients and families on managing their health conditions and building confidence in self-care. Utilize strong clinical assessment skills to monitor patient conditions and identify changes.
Requirements
- Minimum one year of recent clinical experience as a Registered Nurse recommended.
- Home care experience recommended.
- Adaptability, compassion, and a commitment to patient-centered care.
- Ability to work independently and manage time effectively.
Responsibilities
- Conduct thorough assessments and develop individualized care plans in collaboration with physicians, patients, and families.
