About The Position
This is a full-time Registered Nurse position based in a clinic setting. The role involves conducting assessments, coordinating care, developing care plans, and maintaining accurate documentation. The nurse will also participate in 'on call' coverage and attend various team and care plan meetings. The position requires a Registered Nurse with CPR certification, three years of nursing experience, and at least one year of experience in home care, geriatrics, case management, or discharge planning with a frail elderly population in various settings. The nurse must be in good standing with Medicare and Medicaid programs.
Requirements
- Registered Nurse and be a graduate of a school of professional nursing.
- Certified in CPR
- Three years' experience in nursing
- Minimum one year nursing experience with home care, geriatrics, case management, or discharge planning with frail elderly population in acute long-term care, or community-based setting.
- In good standing with the Medicare and Medicaid programs
Responsibilities
- Conducts assessments for new enrollees and periodic reassessments for current participants.
- Coordinate with Home Care Coordinator to ensure CSC participants receive appropriate home care.
- Contributes to the development and revision of participant care plans.
- Completes Nursing documentation and ensures completion, accuracy and relationship to priorities and goals.
- Participates in providing 24/7 'on call' coverage in collaboration with other professional staff, as assigned.
- Attend CSC team meetings, care plan meetings, and including scheduled meetings with participants and/or families.
