About The Position

The Registered Nurse works in the Acute Rehab department at Mercy Hospital in Oshkosh, Wisconsin. This part-time, night-shift role is responsible for coordinating and delivering individualized patient care to support recovery and achieve high-quality outcomes. The nurse collaborates with the interdisciplinary team and clinical and therapeutic disciplines throughout the patient's stay, from admission through discharge.

Requirements

• Registered Nurse credential obtained before the hire date or job transfer date.
• BLS Provider credential obtained within 30 days of the hire date or job transfer date; American Heart Association or American Red Cross credentials are accepted.
• Diploma from an accredited school or college of nursing, or required professional licensure at the time of hire.

Nice To Haves

• Approved Local Exception; the posting notes an exception for Tennessee to add NRP and STABLE, or use for unique union job requirements.
• Driver's License.

Responsibilities

• Lead the execution and continuous monitoring of individualized patient care plans to support recovery progress and high-quality outcomes.
• Document patient conditions and care thoroughly, and communicate critical updates to the interdisciplinary team for timely intervention.
• Coordinate clinical and therapeutic disciplines to provide a seamless, cohesive, patient-centered experience from admission through discharge.
• Carry out physician and nursing orders promptly and precisely to support patient safety and adherence to treatment.
• Assess discharge needs early and collaborate with the care team on transition plans for a safe continuum of care.
