About The Position

The Registered Nurse Acute Rehab role is based at Ascension Alexian Brothers in Elk Grove Village, Illinois, and is a part-time, day-shift position. The nurse implements and monitors patient care plans, monitors and communicates patient condition, coordinates care across disciplines, carries out physician and nursing orders, and coordinates discharge planning with the healthcare team. A diploma from an accredited school or college of nursing, or the required professional licensure at the time of hire, is needed. An Illinois Registered Nurse credential is required before hire or transfer, and BLS Provider certification is required within 30 days of hire or transfer. Previous hospital bedside experience is needed.

Requirements

• Registered Nurse credential from the Illinois Board of Nursing, obtained before the hire or transfer date.
• BLS Provider certification obtained within 30 days of hire or transfer; American Heart Association or American Red Cross certification is accepted.
• Diploma from an accredited school or college of nursing, or required professional licensure at the time of hire.
• Previous hospital bedside experience.

Responsibilities

• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate.
• Serve as the primary coordinator of all disciplines to support well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient discharge planning needs with members of the healthcare team.
