About The Position
DaVita is seeking an organized, self-directed Registered Nurse to manage peritoneal dialysis (PD) patients. The role combines clinical care, patient education, and case management. The nurse educates patients and caregivers in the clinic so they can perform dialysis independently at home, and supports safe, high-quality outcomes through ongoing monitoring and collaboration. The schedule is Monday through Friday, with 8-hour shifts and weekends and holidays off. Scheduling is flexible to support management of the nurse’s caseload. The role includes home visits and on-call support.
Requirements
- Hold an RN license in the state of practice.
- Have a valid driver’s license, insurance, and reliable transportation.
- Have an associate degree in nursing (ADN).
- Have basic computer skills, including Microsoft Word and Outlook.
Nice To Haves
- A Bachelor of Science in Nursing (BSN).
- Dialysis experience, including PD, HHD, chronic, or acute dialysis.
- Patient education or teaching experience.
- Case management or telehealth experience.
- CNN or CDN certification.
Responsibilities
- Train patients and caregivers on peritoneal dialysis procedures and self-care.
- Manage a caseload of peritoneal dialysis patients, including conducting assessments and developing care plans.
- Monitor treatment progress, conduct home visits, and provide on-call support.
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes.
