Beth Israel Lahey Health•Winchester, MA

About The Position

The Patient Flow, Admissions & Discharge Registered Nurse is responsible for assessments and tasks that support smooth patient movement and care coordination. Under general supervision, the RN coordinates and completes admissions and discharges on designated patient care units according to hospital needs. The role supports safe, efficient, patient-centered transitions by completing admission assessments, medication reconciliation, patient and family education, and related unit-workload tasks. It helps reduce bedside nurses’ admission and discharge workload so they can focus on direct patient care, while supporting operational efficiency and patient flow. The RN completes admission and discharge documentation, coordinates discharge planning with nursing supervisors, providers, bedside nurses, and the interdisciplinary team, and ensures discharge instructions, prescriptions, follow-up information, and transportation plans are addressed. The position uses the nursing process to assess patients, develop and implement individualized plans of care—including discharge plans—and evaluate and revise those plans as appropriate. The RN administers ordered medications and treatments, participates in care conferences and performance improvement activities, and completes unit-specific competencies. Schedule and travel requirements are based on department needs. The role reports, in most cases, to a Director/ACNO, Nurse Manager, Nurse Coordinator, or Supervisor and does not supervise others.
