About The Position
Aveanna is seeking a full-time Registered Nurse for its Home Health team in Alachua County. The nurse provides individualized, one-on-one skilled nursing care in patients’ homes to promote recovery, safety, and independence. The role includes assessing patients’ physical, emotional, and environmental needs; developing patient-centered care plans; and delivering skilled interventions tailored to each patient’s goals and condition. The nurse educates patients and caregivers, monitors progress and changes in condition, communicates with physicians and the interdisciplinary care team, coordinates services, and completes timely, accurate clinical documentation. The position follows a Monday–Friday schedule with a shared on-call rotation. Requirements
- Active Registered Nurse (RN) license in the state of practice.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver high-quality care.
- Current driver’s license, reliable transportation, and auto insurance.
- CPR certified or able to obtain certification.
Responsibilities
- Provide individualized, one-on-one skilled nursing care in patients’ homes.
- Conduct comprehensive in-home assessments of patients’ physical, emotional, and environmental needs and develop patient-centered care plans.
- Provide skilled nursing interventions, including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management.
- Educate and support patients and caregivers in managing symptoms, medications, and daily care at home.
- Monitor patient progress, identify changes in condition, and communicate with physicians and the interdisciplinary care team.
- Coordinate care across providers and services to support continuity and quality outcomes.
- Complete timely, accurate documentation that supports quality, compliance, and measurable outcomes.
