About The Position
Providence at Home, a division of Providence Life Services, is seeking a full-time Home Health Registered Nurse to provide home-based nursing care in the Palos Heights, Illinois area. Home health visits take place within a 25-mile radius of Palos Heights. The organization is Joint Commission accredited and has provided home-based services for 30 years. The schedule is Monday through Friday during normal business hours. On-call duties are compensated additionally, and mileage is reimbursed at $0.76 per mile. The listed salary is $85,000–$88,000 per year. The nurse assesses patients and families, evaluates nursing needs, develops and revises plans of care, and provides preventive and rehabilitative nursing procedures while including patients and families in care planning.
Requirements
- Must be a Registered Nurse licensed in the State of Illinois.
- Minimum of 1–2 years of experience, including at least one year in home care nursing, hospice, the emergency room, or medical-surgical nursing.
- Must be CPR certified.
Responsibilities
- Complete initial assessments of patients and families to determine home care needs.
- Conduct a complete physical assessment and gather the history of current and previous illnesses.
- Regularly re-evaluate patients’ nursing needs.
- Initiate and revise the plan of care as patient status and needs change.
- Use health assessment data to determine nursing diagnoses.
- Develop care plans with goals based on nursing diagnoses and therapeutic, preventive, and rehabilitative nursing actions.
- Include patients and families in the care-planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
