About The Position

FreedomCare is a healthcare company dedicated to revolutionizing the home care industry since 2016, supporting patients by empowering them to choose a caregiver for their home care needs. This is a remote Registered Nurse (RN) position for the Georgia team, requiring the candidate to reside in Georgia. While remote, occasional visits to Georgia offices may be necessary. The role involves clinical evaluations for Adult Foster Care members to assess needs for enrollment, service plan development, and ongoing compliance. Monthly visits are conducted for members on care to ensure quality service and address condition changes.

Requirements

  • Registered Nurse licensed by Georgia Board of Registration in Nursing
  • Minimum of 2 years clinical experience
  • 1 year of experience with elders or adults with disabilities
  • Valid driver’s license with an insured vehicle for travel within the state
  • Strong communication and interpersonal skills.
  • Problem-solving and decision-making abilities.
  • Organizational and multitasking skills.
  • Excellent communication, critical thinking, and organizational skills
  • Self-motivated
  • Able to thrive in a fast-paced start-up environment

Nice To Haves

  • Bilingual in Spanish is a plus
  • Experience with CRM (Salesforce is a plus)

Responsibilities

  • Provides support to the caregiver and member, and reviews services according to scope of Adult Foster Care.
  • Respects client and employee rights and privacy, ensures the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
  • Follows the FreedomCare’s clinical policy and procedures.
  • Assesses and regularly reassesses the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.
  • Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.
  • Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
  • Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.
  • Interprets the philosophy and objectives of the agency to clients, families, and others.
  • Performs other job-related duties as may be required or directed by the supervisor.
