About The Position
Family Health and Rehab is a distinguished Skilled Nursing Facility dedicated to providing residents with a homelike environment that promotes independence and well-being. Our facility features private bedrooms, warmly decorated communal areas, and a committed rehabilitation team focused on enhancing quality of life. We are seeking a dedicated and experienced Registered Nurse to lead our nursing team in delivering exceptional patient care. The ideal candidate will possess strong clinical expertise across various specialties, including critical care, long-term care, and acute care settings. As a RN, you will oversee daily nursing operations, coordinate patient assessments, and ensure compliance with healthcare standards while fostering a collaborative and compassionate environment.
Requirements
- Extensive background in long-term care facilities including nursing home experience, memory care, dementia care, and assisted living environments.
- Hands-on experience with EMR/EHR systems like Point Click Care is highly desirable.
- Demonstrated competence in managing complex cases involving infectious disease care, hospice & palliative medicine, occupational health, and pain management.
- Strong knowledge of HIPAA regulations along with proficiency in medical terminology, anatomy knowledge, and physiological assessment techniques.
- Ability to work effectively with diverse populations including geriatrics, individuals with developmental disabilities or behavioral health needs.

Responsibilities
- Provide clinical leadership by supervising nursing staff and coordinating patient care activities across multiple departments.
- Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to inform individualized care plans.
- Administer medications, injections, IV infusions, and perform catheter care following aseptic techniques and medical documentation standards.
- Oversee the use of EMR (Electronic Medical Records) systems such as Point Click Care to ensure accurate record-keeping.
- Manage acute pain cases, facilitate triage procedures, and coordinate emergency responses within the facility’s Level I or Level II trauma center environment.
- Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes.
- Ensure infection control protocols are strictly followed to maintain a safe environment for residents and staff.
- Support staff development through training in specialized procedures such as, ostomy care, gastrostomy feeding, and laboratory specimen collection.
