About The Position

The Registered Nurse (RN) On Call Home Health position is responsible for the assessment, planning, implementation, and evaluation of total nursing care for assigned patients on an intermittent visiting basis. This role typically operates in a home care setting and requires the RN to utilize communication, critical thinking, problem-solving, and decision-making skills to develop patient care plans that consider physiological, psychosocial, and cultural diversity. The RN is also responsible for supervising and delegating tasks to non-professional personnel to ensure optimal patient care and must demonstrate competence in addressing age-related needs of diverse patient populations. Working collaboratively with a multidisciplinary team, including therapy services and medical social services, the RN provides comprehensive care in accordance with Riverside Health System's standards and goals.

Requirements

  • Registered Nurse (Required)
  • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
  • Drives personal vehicle for RHS business 25% or more of the time to perform essential functions of the job
  • Valid Drivers License Required

Responsibilities

  • Assesses, plans, implements, and evaluates the total nursing care of assigned patients on caseload on an intermittent visiting basis.
  • Applies standards of practice as it relates to the specialty or patient population served (e.g., med surg, hospice, IV therapy).
  • Develops the initial and ongoing comprehensive assessment of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate points.
  • Supervises and delegates non-professional staff to ensure optimum patient care.
  • Recognizes and responds to emergency situations.
  • Evaluates own needs for support and uses identified systems and resources to meet patient care needs.
  • Supervises LPN/LVN's, and Home Health Aides in providing services.
  • Completes all required clinical documentation consistently per RHS standards, including completing and 'locking' patient notes and verbal orders, as well as completing certifications and recertifications after department multi-disciplinary team meetings.
  • Completes required documentation no later than the last day of the month to ensure timely billing.
  • Implements/initiates a plan of care and revises when necessary.
  • Provides services and/or treatments requiring specialized nursing skill.
  • Councels the client and family in meeting nursing needs.
  • Coordinates services.
  • Informs the physician and other staff of changes in the client's needs.
  • Plans for the patient to be discharged from services.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient.
  • Participates in case conferences.
  • Provides education to client and family.
  • Participates in training and support of all team members, volunteers, and students as needed.
